Care Management App

Simplify complex care, one tap at a time.

Post-discharge challenge

Families feel isolated and overwhelmed managing their child’s complex care needs. Parents of children with LVADs face a daunting transition from hospital to home care, armed only with a 300-page paper manual and 120 days of training.

Caregivers need supports.

To understand the caregivers (the parents) difficulties, I conducted semi-structured interviews with 3 pediatric cardiac cliniciansaccompanied 2 families on their day-to-day care processes, and held contextual interviews with 4 caregivers in their home settings.

Through the research, I gather insights into 3 categories.

Daily care
  • No immediate support when uncertain about procedures
  • Difficult to find information in extensive documentation
  • Caregivers express constant fear of making mistakes
  • Burden of remembering numerous rules for daily activities
Data management
  • Paper-based recording with no digital solution
  • Doctors can only view data during monthly visits
  • Parents forced to memorize normal biodata ranges
  • No real-time monitoring for caregivers or medical staff
  • Difficult to identify concerning patterns over time
Communication
  • Difficulty remembering troubleshooting procedures during alerts
  • High stress situations impair memory recall of training
  • Limited to verbal descriptions when calling emergency line
